• Max age 20 years old for Pharmaceutical High School (SMF) or equivalent, and max age 23 years old for D3 majoring in pharmaceutical or equivalent
• Copy of academic certificate or SKL (Surat Keterangan Lulus) letter for newly graduated
• Copy of affidavit (Surat Sumpah)
• Copy of STRTTK (Surat Tanda Registrasi Tenaga Teknis Kesehatan) - if any
